 Create a Membership Toolkit Profile for your student - be sure you are logged in so you can access the forms!

 Read the 2025-2026 ACP Policies and Procedures (this information is also laid out in the Student. Enrollment Agreement).

 Complete and submit the Student Information Form.

 Complete and submit the Parent/Guardian – Student Enrollment Agreement.

 Complete and submit the Student Medical Form.

 Complete and submit the Emergency Authorization Form.

 Pay your $75 Registration Fee to secure your spot.

Pay your $180 monthly fee no later than the first day of care.
